Passport Day Event Presented By The CT Passport Agency

Event is open to everyone and passport applications will be accepted at the event.

Wednesday April 11th 4:30 p.m. - 7:00 p.m.

Please contact CA-PPT-CT-Applynow@state.gov for an appointment. Walkins are welcome. Passport applications can be downloaded from the web (DS-11 & DS-82)

To apply for your U.S. passport, you must:

1. Renewing a passport – A completed Passport Application From DS-82
   

2. Submit old passport

3. Provide one recent Passport Photo – 2x2

4. Passport Fees (No Cash)

5. 1st Time application - A completed Passport Application Form DS-11
   

6. Submit your original Birth Certificate or Naturalization Certificate (Your certificate will be mailed to the address on your DS-11 Application)

7. Provide one recent Passport Photo – 2x2

8. Present Valid Photo Identification – (Driver’s License, State Identification Card, Employee ID or Military ID)

9. Passport Fees (No Cash)
